All About NYS' New Excelsior Pass

Excelsior Pass

NYS' new Excelsior Pass is an app that can provide proof of vaccination to permit entry into certain events and facilities in New York. We don't control the app - the State does. But here are some tips that can help you navigate the app and any issues.

Participation in Excelsior Pass is voluntary. If you choose to participate, you can download the Excelsior Pass Wallet app from the Apple App Store or Google Play Store and follow the instructions to retrieve your Pass after you have been fully vaccinated or tested negative for COVID-19 in the State of New York. Alternatively, you may retrieve your Pass from the Excelsior Pass website at From the website, you can print a paper Pass, take a screen shot of your Pass, or save it to the Excelsior Pass Wallet mobile app. More details about retrieving a Pass are available in the "Retrieving a Pass" section of the FAQs.

Sullivan County's only involvement is uploading vaccination records from our clinics to the State's database. If you have received a COVID-19 vaccination at one of the County’s Public Health clinics, be assured that all vaccination information is uploaded at the time of the clinic to the NYS Immunization Registry, from which the Excelsior Pass draws its information.

If you are having trouble with the Excelsior Pass, it may be because it has not been 14 days or longer since your final shot, or you may be having issues with the app itself. While we cannot assist with problems in downloading the app, If you are still having trouble, you can fill out the Excelsior Pass support form or reach out to the Excelsior Help Desk at 844-699-7277 from 7 a.m. to 10 p.m. daily. Note that their support team does not have access to any personal health records and therefore will not be able to provide specific details about personal eligibility for a Pass. In any case, you should keep your paper vaccination card (which you received at our clinics) in a safe place and take a screenshot/ picture of it to keep on your phone.

If you note there has been a mistake in the spelling of your name or date of birth on your vaccination card, that will affect your ability to sign up for an Excelsior Pass. If that is the case, you can call us at 845-513-2240 and leave a message. We'll be happy to assist! (Callers need to be either the vaccinated person or a legal caretaker/guardian of the vaccinated person.)
