Flu Information & Updates

Certified Home Health Agency (CHAA)
Provides home visits by nurses, home health aides/homemakers, physical therapists, speech therapists, occupational therapists, and medical social workers. Medicare, Medicaid, accepted insurance billed and sliding scale.

Diagnostic & Treatment Center 
Includes communicable disease reporting and investigation including rabies prophylaxis, HIV counseling and testing, immunization clinics, childhood lead screening program, sexually transmitted disease program, and tuberculosis control program.


Long Term Home Health Care Program
Maintains nursing home level of care patients in their home as long as possible at a cost of 75% or less of nursing home institutional level of care. Program provides visits by nurses, home health aides, personal care aides, and rehabilitation services. Medicaid, Medicare accepted insurance billing and sliding scale.

 Maternal and Child Health Programs
Sullivan County Public Health Services has a variety of Maternal and Child Health related programs and resources for Sullivan County residents to assist with pregnancy, parenting, or health and social development needs of mothers and children.  Programs include Children with Special Health Care Needs programs (CSHCN),  home visiting programs such as the Community Health Worker program and the Healthy Families of Sullivan, public health nursing services, the Child Passenger Safety program, immunization, HIV testing, lead poisoning prevention, health education, smoking cessation referral to the NYS Fax to Quite line, and WIC.

The Children with Special Health Care Needs Program (CSHCN) includes three programs.  The Early Intervention Program (ages 0-3), the Preschool special education program (ages 3-5), and the Physically Handicapped Children's Program (PHCP, up to age 21). (845) 292-5910 x 2202

Early Intervention Program -Evaluates and tests for developmental delays for children ages newborn to 3 years. Services include speech, occupational and physical therapy, special education, and case management for eligible children. Referrals can be recieved from any concerned parent, health care provider, family member or agency.

Preschool Special Education Services - Currently serves about 250 preschoolers aged 3-5 with disabilities annually.  In addition to acting as a fiscal agent, the Preschool Special Education Program works with school districts and families to develop appropriate Individual Education Plans for preschoolers and to assure that services are delivered in the child's natural environment to the extent possible. Physically Handicapped Children's Program
Identifies children under 21 years of age with physically handicapping conditions and/or correctable physical defects and refers them for diagnosis and evaluation. The program also authorizes and provides payment for specialized medical services for conditions previously diagnosed.

Women, Infants & Children (WIC) Program
The WIC Program provides supplemental foods, nutrition education and counseling, and referrals to health and social services for low-income pregnant, lactating or postpartum women, infants and children less than five years old.  Breastfeeding support is offered to provide information and guidance to women who choose to breastfeed their babies.

Provides consumers and their caregivers with comprehensive and unbiased information about long term care options in Sullivan County.  This program serves the elderly, children with physical disabilities, and adults with physical disabilities -- regardless of payment source -- through a Single Point of Entry for information and assistance.
