Please be aware the Care Center at Sunset Lake is currently CLOSED to visitors, in order to protect residents and staff during the COVID-19 (Coronavirus) outbreak.

To the Families and Loved Ones of Those at the Care Center at Sunset Lake

The dedicated crew of the Care Center would like to thank everyone for their patience and understanding during this challenging time.

First and foremost, we want to acknowledge that we understand that each and every family, friend, and resident has concerns at this time. We want to share with you what steps have been taken, and what will continue to be done, at the Care Center at Sunset Lake.

•           Communication, as you know, has been challenging. We are doing our best to communicate to each and every family member as timely as we can.  Please know that you can always reach out to your loved one’s case manager for information.  Staff will notify families of any change in condition of their loved one. 

•           The NYS Department of Health is currently working closely with the Care Center regarding possible exposures and directing us on what steps need to be taken. You would have already been notified if your loved one was tested. Any additional testing will be done at the hospital, if warranted. If there is another confirmed case, the Department of Health will again give guidance.

•           Here are the steps that we are taking so you can have the most information regarding safeguarding our residents and staff: 

o          Residents

         Daily checks of all residents’ temperatures are taken / monitored every shift (8AM - 4PM, 4PM - 12AM, 12AM - 8AM)

         Social distancing – 6 feet away from each other, and no group activities

         Staff are wearing personal protective equipment (PPE) for the protection of the residents and themselves

o          Staff

         Screened upon start of each shift.  Temperature taken, condition assessed and documented in a secure area before being allowed in the main facility

         PPE is provided and strictly enforced

         Daily staff education and updates

•           In regards to daily operations at the Care Center:

o          We are continuing therapies, although some have been modified to room therapy if it can be accommodated

o          Activities are ongoing.  We are not doing large group activities.  We are meeting people in their rooms, and lots of fun and healthy individualized activities are occurring in the building

o          Meals are being served with no disturbance

o          DPW (Sullivan County Department of Public Works) has increased cleaning and sanitizing the facility

o          Laundry is continuing as usual

The Care Center at Sunset Lake is a skilled nursing facility providing care 7 days a week, 24-hour a day. 

The facility is licensed by the New York State Department of Health and offers skilled restorative care in a therapeutic environment for those who need rehabilitative care as well as a therapeutic environment for those who can no longer live independently. Also known as the Sullivan County Adult Care Center, the facility provides short term placement for restorative care, short term respite stays or long term placement. We also have a memory care unit specialized to the care of those with the diagnosis of dementia. The facility has 146 beds.

Services include; a full array of medical, nursing, personal care, dietary, physical therapy, occupational therapy, speech therapy, social, spiritual, recreational and housekeeping services. Residents and/or families are involved in decisions about care and receive services based on their individual needs.


  • Facility - 7 days a week, 24 hours a day 
  • Administrative Staff - Mon-Fri 8:00am - 4pm 

Eligibility Requirements:

  • 16 years of age or older 
  • Must have a skilled medical need that cannot be addressed in a less restrictive environment 

Cost of Service and Fee Structure:

  • Private Pay Rates (room and board ONLY):
    • $270 per day semi-private room 
    • $280 per day private room 
  • Medicaid is accepted 
  • Medicare may pay for a portion of services IF specific medical criteria are met and for a LIMITED time 
  • Private insurance is accepted if the insurance company agrees that nursing home care is necessary AND the insurance company is willing to pay for the care 
  • Co-pays may apply and are collected

  • Documentation Required:
    • A Patient Review Instrument (PRI) and Screen assesses the applicant's ability to complete "activities of daily living" such as walking, bathing, dressing, etc. The assessment includes all physical and psychological problems the applicant faces. 
    • A financial application 

Intake/Application Process:

  • The PRI and Screen is done by Sullivan County Public Health Services if the individual is living in the community. If the individual is in the hospital or another nursing home that facility will do the PRI and Screen. The financial application is available from the Social Services Department at the Care Center at Sunset Lake (CCASL). Upon review of the PRI and Screen a decision is made as to whether CCASL can meet the health care needs of the applicant
