Adult Day Health Services

Adult Day Health Care Services located at Sullivan County Adult Care Center

Adult Day Health Care Services is designed to meet the medical, physical, mental, emotional and social needs of adults in a congregate setting. The program goal is for the registrant remain in the community with the assistance of the family and/or support services as long as their needs can be met and safety maintained. The registrant is linked with needed services. On-site services include nursing, medical social worker, rehabilitation therapies (physical therapy, occupational therapy, and speech therapy), podiatry and nutrition.

Hours:

Monday through Friday – excludes holidays - 8:30 AM to 2:30 PM

Services Include:

Nursing
Case management and coordination
Social services
Physical therapy
Speech therapy
Therapeutic activities
Assistance with activities of daily living
Blood work and treatments are done as needed

Eligibility Requirements:

21 years of age
Medical necessity
Adults with limitations

Cost of Service and Fee Structure:

Private Pay: $115/day does not include transportation
Medicaid: Medicaid rates vary

Method of Payment Accepted:

Medicaid or private payments

How to Apply/Documentation Required for Service

Call for an appointment for eligibility
A person may attend for one day before making a decision to register

Intake Procedure (Application Process)

Referral is received
A Registered Nurse will assess registrant's needs in the community setting and complete a Registrant Assessment Instrument (RAI)
A physician must sign medical orders
If a registrant receives Medicaid, arrangements are made for transportation through Medicaid
