OFA Senior Identification Card   

Seniors are encouraged to obtain a Senior Identification (ID) card from the Office for the Aging. This is a laminated photo ID that is issued to eligible seniors. This card entitles seniors to use our nutrition and transportation services and other benefits available through our office. To be eligible for an ID card, one must be at least 60 years of age and show proof of age and residence. There is a suggested donation of $5.00 for the photo ID card.  The photo can be taken at our office.
